Goals & Objectives
Pre-Transport
1. Obtain relevant clinical information from referring providers to anticipate appropriate level of care at receiving institution.
2. Provide appropriate diagnostic and therapeutic recommendations to referring providers when clinically indicated.
3. Collaborate with team members to choose the appropriate mode and level of transport based on clinical information, team member availability, and safety conditions.
4. Develop clinical management plan with other team members.
1. Complete a focused clinical assessment of the patient within 5 minutes of arrival at bedside of referring facility.
2. Participate in a patient handoff from referring providers in a professional manner, ensuring all relevant information is received.
3. Accurately categorize level of acuity based on clinical assessment.
4. Collaborate with transport team members to develop a plan of care.
5. Identify situations in which expert consultation is indicated.
6. Initiate required interventions for stabilization of patients prior to transport.
7. Apply principles of family-centered care to transport setting, in particular
(a) treating the patient and family with respect and dignity
(b) sharing complete and unbiased information with the patient and family in a way they find useful and affirming.
Intra-Transport
Post-Transport
1. Demonstrate skill in the use of handoffs to communicate about the patient’s condition with receiving providers. (I-PASS, SBARQ, etc.)
2. Give examples of communication failures between transport team and receiving providers during handoffs. List methods to prevent these.
3. Accurately document consultations, interventions, medical decision making in the medical record to support a safe transition of care.
4. Recognize the importance of, and participate in, team debriefing and case review with regard to quality improvement, patient safety, and individual and team development.
